الندوة أقيمت بمناسبة اليوم العالمي للسرطان 2023 واستعرضت التقدم الذي حققته منطقة شرق المتوسط في رعاية مرضى السرطان

تحت شعار حملة اليوم العالمي للسرطان لعام 2023 “توحيد الأصوات واتخاذ الإجراءات”، عقدت الجمعية الأمريكية لبحوث ومصنعي المنتجات الدوائية (PhRMA) في منطقة الشرق الأوسط وإفريقيا، والتحالف الدولي لمنظمات المرضى (IAPO) وتحالف الرعاية المخصصة للسرطان (PCCA) ندوة حوارية عبر الإنترنت لمناقشة دور الرعاية المخصصة للسرطان في بثّ أمل جديد للمرضى.

انعقدت الجلسة يوم الأربعاء 1 فبراير 2023، وتناولت إسهام الطب الدقيق والرعاية المخصصة للسرطان في إحداث ثورة في علاج السرطان، وكيف يؤثر ذلك في طريقة توفير الرعاية للمرضى وتحسين جودة حياتهم.

شارك في النقاش عبر الإنترنت الدكتور غاي فونز، رئيس آلية التنسيق العالمية التابعة لمنظمة الصحة العالمية بشأن الوقاية من الأمراض غير السارية ومكافحتها؛ وهناء شعار شعيب، عضو مجلس إدارة الاتحاد الدولي لمكافحة السرطان، ومدير عام مركز سرطان الأطفال في لبنان؛ وكوالديب سهمي، المدير التنفيذي للتحالف الدولي لمنظمات المرضى؛ وأبيغيل أميت، مديرة الابتكار وتفاعل المرضى لمنطقة شرق أوروبا والشرق الأوسط وإفريقيا، في شركة “ميرك شارب ودوم” ورئيسة تفاعل المرضى في الجمعية الأمريكية لبحوث ومصنعي المنتجات الدوائية (PhRMA) في منطقة الشرق الأوسط وإفريقيا. وتناول المتحاورون دور الرعاية المخصصة للسرطان والطب الدقيق في تمكين المرضى من المشاركة بفاعلية أكبر في مسارات علاجهم. ويهدف هذا النموذج إلى توفير العلاج في الوقت المناسب بما يخدم الهدف النهائي في تحقيق أفضل النتائج، وتحسين سلامة المرضى، والحدّ من مخاطر الانتكاس.

وبهذا الخصوص، قالت جيرترود إيكارت، رئيسة الفريق المعني بسياسة الأورام لمنطقة شرق أوروبا والشرق الأوسط وإفريقيا، في شركة “ميرك شارب ودوم” ورئيسة فريق تنفيذ سياسة الأورام في لمنطقة الجمعية الأمريكية لبحوث ومصنعي المنتجات الدوائية في شرق أوروبا والشرق الأوسط وإفريقيا: “يعدّ مرض السرطان أحد الأسباب الرئيسية للوفاة في العالم، فقد أزهق أرواح نحو 10 ملايين شخص في عام 2020، أو ما يعادل نحو وفاة من كل 6 وفيات[1]. وتبشر الرعاية المخصصة للسرطان المقترنة بأساليب الطب الدقيق، بتحسين قرارات العلاج للمرضى المصابين بأنواع معينة من السرطان”.

وفي إطار إدارتها للندوة الحوارية التي انضم إليها أكثر من 200 مشارك من أنحاء العالم، أوضحت إيكارت أن مؤسستها تسعى جاهدةً للتوعية بنموذج الرعاية الصحية المخصصة، واستعراض مزاياه العديدة للمرضى والأطباء و أنظمة الرعاية الصحية.

وقال كوالديب سهمي، المدير التنفيذي للتحالف الدولي لمنظمات المرضى: “يعد التقرير الأخير عن الرعاية المخصصة للسرطان الأول من نوعه لتسليط الضوء على الرعاية المخصصة لمرضى السرطان، وتضمن مقالات موسعة لكوكبة من العلماء والقادة البارزين في هذا المجال، فضلاً عن توصيات لوضع السياسات الكفيلة بتحسين مشهد الرعاية المخصصة للسرطان في المنطقة عموماً. ويشكل هذا التقرير خارطة طريق شاملة للمضي قدماً في توفير خدمات الرعاية المخصصة لجميع مرضى السرطان في البلدان منخفضة ومتوسطة الدخل”.

وأوجز سهمي مفهوم مؤسسته لإدارة الرعاية المخصصة للسرطان؛ والذي يقوم على توفير خدمات رعاية تنبؤية ووقائية ومخصصة وتشاركية واستباقية للمرضى. وترتبط الرعاية التنبؤية بتحديد الأشخاص الأكثر عرضةً للإصابة بالسرطان؛ وفي المقابل تركّز الرعاية الوقائية على إطلاق حملات التوعية الصحية؛ أما الرعاية التشاركية، فتهدف إلى ضمان مشاركة المرضى مشاركةً فاعلة في خططهم العلاجية؛ وتُعنى الرعاية المخصصة باستخدام أحدث التقنيات لتوفير علاجات مخصصة؛ وأخيراً تسعى العناية الاستباقية إلى تمكين النظم الصحية من معالجة حالات الإصابة قبل تدهورها.

سلط سهمي الضوء كذلك على أحدث مقترحات التحالف لمنطقة شرق المتوسط تحديداً، وهي منطقة حددتها منظمة الصحة العالمية وتضم 22 دولة في منطقة الشرق الأوسط وشمال إفريقيا – تتنوع من الدول الغنية بالنفط إلى البلدان النامية الفقيرة. ودعا سهمي إلى توحيد الجهود لرعاية مرضى السرطان البالغ عددهم نحو 730 مليون مريض في هذه المنطقة.

وأردف سهمي: “تحتفل منظمة الصحة العالمية هذا العام بالذكرى السنوية الخامسة والسبعين لتأسيسها. وهي فرصة رائعة لتقييم جهودنا لرعاية مرضى السرطان وضبط بوصلتها للسنوات الـ 75 عاماً القادمة من خلال الاستثمار في تقنيات وأساليب الرعاية المخصصة للسرطان، والاستفادة من الطب الجينومي، والرعاية الصحية الرقمية، والذكاء الاصطناعي وتعلم الآلة حتى نتمكن من تشخيص وعلاج السرطان بالشكل والوقت المناسبين”.

ويتزامن الاحتفال باليوم العالمي للسرطان لعام 2023 مع النسخة الثانية من حملة “سد فجوة الرعاية” التي تستمر لثلاث سنوات. وأقيمت الندوة لهذه المناسبة تحت شعار “توحيد الأصوات واتخاذ الإجراءات” بالتزامن مع إصدار التقرير الأخير حول الرعاية المخصصة للسرطان – “الفرص المتاحة للمرضى في منطقة الشرق الأوسط وإفريقيا”.

وقالت هناء شعار شعيب، عضو مجلس إدارة الاتحاد الدولي لمكافحة السرطان ومدير عام مركز سرطان الأطفال في لبنان: “تنعقد ندوتنا الحوارية في إطار الاحتفال بإطلاق حملة ’سد فجوة الرعاية‘ المنضوية ضمن فعاليات اليوم العالمي للسرطان، وتناولنا فيها دور الاتحاد الدولي لمكافحة السرطان في إعداد وإطلاق مبادرات عالمية لتحقيق تكافؤ الفرص في رعاية مرضى السرطان. وتسلط الحملة الضوء في عامها الثاني على ضرورة توحيد أصوات الجميع واتخاذ الإجراءات اللازمة لسد فجوة الرعاية. ومع ذلك، فإن الاحتفال باليوم العالمي للسرطان لا يقتصر على دعوة الأطراف المعنية لتوحيد صفوفهم فحسب، بل يشكل كذلك معركة نخوضها جميعاً لضمان تشخيص وعلاج السرطان بالطريقة والوقت المناسبين، وتحسين تجارب العلاج للمرضى. ونجدد دعوتنا من هذا المنبر لاتخاذ إجراءات الوقاية من السرطان والتوعية به؛ إذ يسهم ذلك في تحقيق النتائج المرجوة بتقليل الوفيات وزيادة معدلات الشفاء على أمل تحقيق الهدف الثالث من أهداف الأمم المتحدة التنمية المستدامة بحلول عام 2030. ويشكل التقرير الأخير عن الرعاية المخصصة للسرطان دراسة مرجعية تسلط الضوء على انعدام المساواة في رعاية المرضى في دول العالم وبينها. ونتطلع هذا العام إلى تحقيق إنجازات قياسية عن العام الماضي آملين أن يساهم تكاتفنا في سد أكبر قدر ممكن من فجوة الرعاية، إن لم يكن كلها، وتوفير تجارب علاجية أفضل لمرضى السرطان حول العالم”.

وشدّد الدكتور غاي فونز، رئيس آلية التنسيق العالمية التابعة لمنظمة الصحة العالمية بشأن الوقاية من الأمراض غير السارية ومكافحتها، على أهمية تخصيص رحلة العلاج بما يلبي متطلبات المرضى ويفي بالتزامات منظمة الصحة العالمية وقال: “نحن بحاجة للانتقال من النموذج الحالي لرعاية مرضى السرطان كأفراد إلى نظام شامل يشارك فيه المرضى كشركاء فاعلين في عملية العلاج. وقد بات من الضروري اتباع نموذج صحي يضمن حقوق الفرد في الحصول على خدمات الرعاية الصحية مع الاستعانة بتجاربهم وخبراتهم ورؤاهم الشخصية حيال المرض لصياغة طيف كامل من الخدمات الصحية عبر مراحل التوعية والوقاية والعلاج والرعاية التلطيفية. وسيضمن ذلك في نهاية المطاف كفاءة واستدامة أنظمة الرعاية الصحية وقدرتها على الارتقاء بصحة الجميع دون استثناء”.

أخيراً تحدث ممثلو الجمعية الأمريكية لبحوث ومصنعي المنتجات الدوائية في منطقة الشرق الأوسط وإفريقيا عن دورهم في إدارة حملة “توحيد الأصوات واتخاذ الإجراءات” لمكافحة السرطان في المنطقة، إذ تشكلت مجموعتا عمل – الأولى للأورام والثانية لمشاركة المرضى – لضمان التعاون الفاعل مع الشركاء حول العالم. وحددت أبيغيل أميت، مديرة الابتكار وتفاعل المرضى لمنطقة شرق أوروبا والشرق الأوسط وإفريقيا، في شركة “ميرك شارب ودوم” ورئيسة تفاعل المرضى في الجمعية الأمريكية لبحوث ومصنعي المنتجات الدوائية (PhRMA) في منطقة الشرق الأوسط وإفريقيا، المهام التي يتعين على مجموعات مكافحة السرطان في المنطقة تنفيذها لضمان نجاح الحملة في عام 2023. واختتمت الندوة الحوارية بفقرة مخصصة لتلقي أسئلة الجمهور.

World Cancer Day 2023 Webinar Discusses how Precision Medicine and Personalised Cancer Care could Herald a Revolution in Regional Cancer Treatment

 

Online discussion held to mark this year’s World Cancer Day outlines advances in cancer care across the Eastern Mediterranean Region  

 

 

Under the 2023 World Cancer Day theme “Uniting Voices and Taking Action” The Pharmaceutical Research and Manufacturers of America Middle East and Africa (PhRMA MEA), the International Alliance of Patients’ Organizations (IAPO) and the Personalised Cancer Care Alliance (PCCA) joined forces to debate how individualised cancer care offers a new promise to cancer patients.

Precision medicine and Personalised Cancer Care (PCC) are revolutionizing the fight against cancer. How this impacts the way patients receive care and their quality of life was subject of discussion during the webinar on February 1, 2023.

The webinar panellists Dr Guy Fones, Head, Global Coordination Mechanism on NCDs, World Health Organisation (WHO), Hana Chaar Choueib, Board of Directors Member at Union for International Cancer Control (UICC) & General Manager at Children’s Cancer Center of Lebanon (CCCL), Kawaldip Sehmi, CEO, International Alliance of Patients’ Organizations (IAPO) and  Abigail Amit, Director of Patient Engagement and Innovation – Eastern Europe, Middle East & Africa – Merck Sharp & Dohme GmbH and PhRMA MEA Patient Engagement Lead, shone a spotlight on how PCC and precision medicine is empowering cancer patients to take a more active role in their therapy.  Established to enable the right treatment to be given to the right patient at the right time, this model of care’s ultimate objective is to provide better outcomes, improved patient safety and a reduced risk of adverse events.

“Cancer is one of the leading causes of mortality worldwide, accounting for nearly 10 million deaths in 2020 – a figure that equates to nearly one in every six deaths.[1]Personalised Cancer Care (PCC) along with precision medicine, offers the promise of refining treatment decisions for those diagnosed with certain types of cancers,” said Gertraud Eckart, Director Oncology Policy – Eastern Europe, Middle East, Africa (EEMEA) – Merck Sharp & Dohme GmbH and Lead of the PhRMA MEA Oncology Policy Task Force.

Hosting the webinar, which saw the participation of over 200 audience logging in from around the world, Eckart explained that her organisation was working hard to spread awareness of personalised healthcare and to generate an understanding of the numerous benefits that it confers for patients, health care professionals and healthcare systems.

Kawaldip Sehmi, CEO of the International Alliance of Patients’ Organizations (IAPO), “The Position Paper ‘Personalised Cancer Care in the Eastern Mediterranean Region’ is a first of its kind to put Personalised Cancer Care in the spot light with thought pieces from scientists and key leaders in the field as well as policy recommendations targeting the overall PCC scene in the region. This paper was produced with the help and support of patients’ organizations and civil society in the region, making it a seminal reference when it comes to Personalised Cancer Care with “actual” patients’ voices being integrated within the paper. It provides a comprehensive road map and a way forward to make sure that Personalised Cancer Care will be accessible to all patients in low- and middle-income countries.”

Sehmi summed up the alliance’s concept of PCC with ‘5 Ps’ – predictive, preventative, personalised, participatory and pre-emptive. Predictive relates to better identification of those at risk of developing cancer; preventative involves health education and promotion campaigns; participatory means ensuring that individuals are actively involved in their cancer care; personalised covers using the latest technology to deliver tailored treatments; and pre-emptive focuses on empowering health systems to tackle situations before they deteriorate.

He also highlighted the Personalised Cancer Care Alliance’s latest proposals for the EMR, a region designated by the World Health Organisation (WHO) comprising 22 member states in the Middle East and North Africa – from affluent oil-rich states to impoverished developing countries. He used the discussion to call for unity in delivering cancer care to the region’s roughly 730 million people.

“This year the WHO marks its 75th Anniversary. It’s a great time to evaluate and re orientate our cancer care for the next 75 years by investing in personalised cancer care health technologies and approaches, exploiting genomic medicine, digital healthcare, artificial intelligence and machine learning so that we can get our cancer diagnosis and its treatment for the right patient, at the right time, and treating it with the right treatments,” Sehmi added.

World Cancer Day 2023 is the second year in a three-year ‘Close the Care Gap’ campaign. For this year’s edition, the webinar addressed 2023 theme of “Uniting our voices and taking action”, by introducing the recent report on Personalised Cancer Care (PCC) – opportunities for patients in Middle East and Africa.

As part of commemorating World Cancer Day’s “Close the Care Gap” campaign, the webinar addressed the role of the Union for International Cancer Control (UICC) in implementing and creating global initiatives for reducing inequity in cancer care. In its second year, it highlighted on everyone’s important role of uniting their voices & to take action.

“The World Cancer Day is not only to raise the voices and shed light on the day but also it is a fight to achieve.  It is a fight to ensure proper diagnosis and treatment for the right patients, at the right time, at the right locations, and make the experience as good as possible for them. It is the call to take action to prevent and educate about cancer. This will yield the right outcomes desired of reducing mortalities and increasing cure rates, hopefully to reach the UN agenda 2030 targets of Sustainable Development Goal 3. It is the call to build partnerships and join efforts. It is a reference report for the cancer community to highlight on the longstanding inequities in cancer care within and between countries across the world. We are aiming this year to break last year’s records of activities and hopefully with joining hands to close enough of the care gap, if not all, and offer better journeys with cancer for patients worldwide.” said Hana Chaar Choueib, Board of Directors Member at Union for International Cancer Control (UICC) & General Manager at Children’s Cancer Center of Lebanon (CCCL).

Dr Guy Fones, Head, Global Coordination Mechanism on NCDs, World Health Organisation (WHO) outlining the importance of the individual with lived experience perspective and the WHO’s commitment on this topic said, “We need to move from the current cancer care model of treatment being something that is done to individuals and instead move to a holistic system that meaningfully engages and better integrates those with lived experience of cancer as active partners in the process. It is essential to ensure a rights-based approach to health that involves using their individuals’ lived experience, expertise and perspectives to shape the full spectrum of our health services, across promotion, prevention, treatment and palliative care. Ultimately, this will ensure that our healthcare systems are effective, efficient and sustainable and crucially can improve health for all.”

Lastly, the discussion included a PhRMA MEA representative, who highlighted how the private sector is leading the ‘Uniting our voices and taking action’ drive in the fight against cancer in the region. PhRMA MEA’s Oncology Working Group and Patient Engagement Working Group has been set up to collaborate with partners worldwide. Abigail Amit, Director of Patient Engagement and Innovation – Eastern Europe, Middle East & Africa – Merck Sharp & Dohme GmbH and PhRMA MEA Patient Engagement Lead, outlined what the different cancer groups in the region need to do to succeed in 2023, with the webinar being rounded off with a question-and-answer session with the audience.
